Long Term Disability Insurance Plan Annual Enrollment

FIT offers a Long Term Disability (LTD) Plan through The Hartford. LTD Insurance Plan coverage replaces a portion of your pay if you are unable to work due to illness or injury, and you apply for and are approved for LTD benefits by The Hartford, the LTD plan insurer.  For complete details about LTD Plan coverage, please refer to the Long-Term Disability Plan Certificate of Insurance (.pdf).  The plan offers four options, and during the Annual Benefits Open Enrollment period you may change to a new option.  Option A is paid for by FIT.  Enhanced Options B, C, or D have a higher benefit level, a shorter benefit elimination period*, or both.

* The Elimination Period is the minimum period you must wait from the start of an approved medical leave until LTD Plan benefits may become available to you.
** Base salary in effect just before your date of disability.

? What you need to do by Friday, November 12, 2021

  • You do not want to make a change:  Do nothing; the current Plan Option elected will carry forward to 2022.
  • You want to change your option: Email a benefits representive using your FIT email address to obtain each LTD's option's associated cost and to request an LTD election form.  Please include your FIT employee ID number.  You must then complete the LTD Election Form to request a change to your LTD plan option. 

    If you complete an LTD Election Form and have selected a higher-level enhanced option for calendar year 2022 than the option you are currently enrolled in, a health questionnaire will be provided to you, which must then be submitted directly to The Hartford for their approval determination.  If approved by The Hartford, they will notify you and the Benefits staff at FIT of the effective date of coverage, and required contributrions will then be deducted from your paycheck.  Coverage may not become effective until approved by The Hartford.
    Additionally, if you select a higher level enhanced option for calendar year 2022 than the option you are currently enrolled in and experience a disability that is caused by, contributed by, or resulting from a pre-existing condition, there is a 12-month waiting period before the new benefit option may be received; you may be eligible to receive the previous, lower benefit during the 12-month waiting period.  A pre-existing condition is defined as a condition for which you received medical care or treatment in the three months before the date the increase in coverage becomes  effective.
